Yes. It would be preferable if the designers did it themselves, but not necessary.

Maybe, but it hasn't happened that I know of.
The game should be terminated or the board should be Failed. I think it's fair to say there was consensus with those reviewers who participated.

that's a true statement. although in practice i think it tends to be one of the following scenarios:
1- its not even worth starting a game (this tends to happen with new members creating a board for the first time and don't realize the merits/requirements of what submitting means).
- this ends up with some feedback to the creator and failure of the game before it starts. then we have convey to the creator how to figure out how to terminate the game (and sometimes the creator bugs out when faced with criticism, no matter how positive/constructive it is)
2- we realize in mid-game the board isn't going to work
- in which case all the players in game come to a consensus to keep playing or not.
3- we play it out to make sure end game is good, game ends up finishing.
- we come to a consensus to pass/fail

in scenario 3 none of this discussion matters.
in scenario 1&2 if its decided we shouldn't play/keep playing, we have to convey to the creator how to terminate the board. so I don't see an issue with combining them as long as the review team has due diligence before failing (which i think we do).
i can see an argument for not coupling the failure of review with termination of game; but on the other hand, if it's not coupled i'd like to see any member of the review team being able to terminate the game.
Yeah, I agree WT. If we want to fail the board, but don't want to terminate the game, we can just wait to fail until the game is over.
Ideally, the review board should have the power to terminate a review game once the board has been failed. If that's too complicated, then the review game should automatically terminate when the board is failed.
Unfortunately (or for whatever reason), reviewers who would otherwise fail the board tend to decline playing the board. As I type this, I'm realizing that my declining to play is sometimes a non-confrontational way of saying "This board is not worth the time to review; please withdraw it from consideration." With hind-sight this doesn't seem like a healthy way to address these situations.

(this was the case with the latest failed board. i didn't have the time to make the tactful words the creator needed, so i PMed another that has in the past offered those.)
